Provides comprehensive programs in an outpatient setting to accommodate the individual needs and unique personal situations. Offers two levels of care which accommodates not only the treatment needs of the individual at the time of the initial intake or transfer, but other circumstances, such as work schedules. The Intensive Outpatient Program (IOP) is a level of care designed for individuals 18 years and older who require a more intensive treatment structure. There is a daytime and evening IOP to accommodate the schedule of the individual. The IOP occurs four times per week and lasts for approximately four weeks. Some of the services include: psychoeducational therapy on addiction and relapse prevention, weekly individual counseling, daily group counseling, and multi-family counseling group. The Outpatient Program is a level of care designed for individuals 18 years and older who require ongoing support for their recovery. Some of the services include: individual counseling, weekly group counseling, week

Inpatient addiction treatment is designed for individuals experiencing symptoms related to acute or chronic drug and alcohol symptoms. Treatment program is highly specialized and takes into consideration each person's unique medical circumstances as well as individual lifestyle and specific obstacles to recovery. Services may include: drug and alcohol screenings and evaluations, full detoxification services, individual therapy, group therapy, recreational therapy, educational therapy, specialized program for healthcare professionals, specialized program for uniformed professionals, specialized services for dual-diagnosis with a mental illness, separate groups for men and women, trauma & PTSD support groups, relationship issues support (including sexual compulsivity/addiction), nutrition education program (including eating disorders), relapse prevention support group

Provides outpatient medication-assisted treatment for opiate addiction, supported with a full complement of clinical and case management services. Provides patients with an environment to help break the cycle of addiction. Men and women struggling with opioid use disorders will find help through General Outpatient Treatment (OP) and Office-Based Opiate Treatment (OBOT) programs. Medication-assisted treatment includes methadone, buprenorphine (Suboxone), and Vivitrol. Treatment plans include individual, group, and family counseling coupled with Acceptance and Commitment Therapy (ACT), Cognitive Behavioral Therapy (CBT), Motivational Interviewing (MI), grief/loss therapy, art therapy, vocational counseling, and life skills training. Relapse prevention and off-site 12-step meetings support long-term recovery. Pregnant women welcome.

Rehabilitation consists of treating the disease of chemical dependency (CD), which requires physical, mental and spiritual rehabilitation. Through daily group and individual counseling, patients develop understanding and self-awareness making recovery possible. Average length of stay for rehabilitation is based on the individual need of each client. Long-term specialty programs are also available.

Provides outpatient treatment and programs for persons with co-occurring mental health and substance use disorders, DUI/DWI offenders, and criminal justice clients.

Provides a full service drug testing service including: drug detection assessments, instant on-site drug testing, DNA and paternity testing, workplace drug testing programs, hair follicle drug testing, in-home drug testing, steroid testing, laboratory testing, and background checks.

Offers a Medication-Assisted Treatment (MAT) program for those suffering from substance use disorders. Team consists of two psychiatrists, a Mental Health Nurse Practitioner, a Psychiatric Nurse Case Manager, a Licensed Practical Nurse, a Registered Medical Assistant and several therapists who are Certified Advance Alcohol and Drug Counselors (CAADC). The program is a combination of both FDA-approved medications and counseling. Three types of MAT are offered:
Vivitrol: a monthly opioid blocker for patients free of opioids for at least five days. We can provide an outpatient detox protocol if needed.
Sublocade: Monthly Buprenorphine injection. Patients need at least seven days on oral Buprenorphine which is provided.
Probuphine: Six-month Buprenorphine implant for patients stabilized on 8mg or less of oral Buprenorphine.
Please note, our practice does not prescribe maintenance oral Buprenorphine.

Also offers prevention and education programming within schools and throughout the community, Narcan training and distribution, and options for the safe disposal of unused and unwanted medication.

Provides substance abuse assessments and outpatient counseling for adults and adolescents. Provides a range of services such as an intensive adolescent mental health counseling program to include group, individual and family counseling with transportation provided.

Provides short, moderate and long term non-hospital residential treatment for male adults who are experiencing problems with chemical dependency.

Offers adult substance abuse counseling on an outpatient and intensive outpatient basis. Provide a comprehensive assessment, referrals, drug and alcohol education, and individual and/or group counseling.

Offers Medication Assisted Therapy (MAT) for Opioid Addiction. Physicians use Suboxone and Vivitol at treatment modalities.

Provides a recovery pathway that provides intensive support and behavioral health case management for pregnant individuals, or new moms, who have a diagnosis of substance use disorder (SUD). The goal of Free2BMom is to improve outcomes for mom and baby by reducing stigma and helping new moms or pregnant individuals face all the normal challenges of being a new parent. As well as overcome other obstacles of navigating care, including access to physical and behavioral healthcare, treatment for SUD, transportation, and connection to community resources. Enrolled individuals will receive case management for a period up to 2 years by a master’s level social worker (BHCM) and a certified recovery specialist (CRS).

Operates an outpatient primary medical care clinic for Veterans through the Lebanon VA. Offers primary care and annual physicals by appointment, preventive services, women's health services, routine lab work, EKGs, immunizations, medication review, tele-mental health and social work services, treatment of conditions such as diabetes, hypertension, respiratory disease, common cold, influenza, tobacco cessation and specialty referrals.

Provides an in-home clinical & case management service designed to eliminate the destructive impact of substance abuse problems within families. Comprehensive and coordinated intensive clinical and case management services are provided to families in which children or adolescents are identified as being at risk of child maltreatment, due to substance abuse issues within the family. Any family member or caregiver may be identified as experiencing a substance use disorder. Primary outcome benefits are alleviation of the risk of abuse and neglect of a child or adolescent and the elimination of substance abuse behaviors for the affected individual.

The medical detox unit uses numerous different medications to effectively make someone as comfortable as possible and finally give the mind and body the rest and nutrition it needs.

Provides prevention education (alcohol, tobacco, opioids and other drugs) and life skills training to help people of all ages learn to make healthy decisions and increase their resiliency for managing change and life's challenges.

Assist individuals experiencing issues related to substance abuse or addiction. Drug and Alcohol services can include diagnostic assessment, individual, group, couples and family therapy. Levels of Care Assessments are completed as requested by the court system as well as those seeking services voluntarily. Treatment in the Drug and Alcohol Program can include various interventions based upon the needs of the participant.

Offers drug and alcohol addiction recovery services in a home setting to individuals age 19 and older. Program includes 12-step meetings, individual and group counseling, and relapse prevention education. Individual residents can stay for as long as 24 months or until they find employment.

Serves as an outpatient treatment facility for adults and adolescents with drug and alcohol addiction as well as children and teens recovering from trauma or are involved in the child welfare system.
Length of treatment is individually determined, based on goals and progress and includes both individual and group therapy sessions.
