Provides an in-person Functional Eligibility Determination (FED) to determine eligibility for Medical Assistance Long Term Services and Supports, both in the community or a facility.

Offers a continuum of inpatient psychiatric care across the life span including: a designated Crisis Response Center (CRC) providing 24/7 assessments; specially-designed inpatient units for adolescents, adults, and older adults (with and without co-occurring substance use issues); and a private supportive residential program for adults and older adults.

Provides elderly or chronically ill veterans with coordinated care that focuses on individual needs, enabling each patient to enjoy the highest quality of life possible. Provides assessment, prevention, stabilization and rehabilitation to eligible residents. Services for Older Persons provides care management and overall guidance to persons age 65 and older to help them maintain maximum personal independence in their own homes. Also helps older adults locate quality assisted living and nursing home placements. Services offered in the five-county, greater Philadelphia region.

Provide care managers to advocate for the individual and work with family members, legal professionals, and financial advisors to understand the individual's specific challenges. Offers support to navigate the health care system, understand health insurance benefits, and determine residential options. Provides education with obtaining referrals when needed.

Provides seniors an in-home service that provides care management services, health services, and provides financial services to the individual.

Provide geriatric assessment of memory issues, dementia, medication reviews, falls, behavioral issues, frailty and maximizing functional ability. Services include geriatric assessments, medication reviews, memory evaluations and preventive care.

Provides wellness classes and activities for older adults.

Provides information and referral, service coordination, and educational programs for older adults living on their own or in assisted care. Geriatric specialist on staff meets clients by phone, at the agency, or at client homes for assessments. Fees apply. Medicare accepted.

Geriatric outpatient assessment center; that provides consultations, evaluations, and recommendations based on the individuals medical history, health condition, current medicines and needs.

Participants will meet with a physician and a social service person for an evaluation of their physical, emotional and cognitive health. The senior will be given a list of recommendations that the team has made to share with their personal physician. The purpose of the program is to educate seniors and their families about services and programs that can help them continue to be active and independent in their communities.

St. Mary Medical Center's partnership with Catholic Housing & Community Services has established the Family Caregiver Center to connect seniors to aging in place resources and services. Our guidance provides older adults and their families with choices, recommendations, and answers in times of uncertainity.

Senior Assessment Program at the Geriatric Center provides comprehensive evaluations performed by a geriatrician, a psychiatrist, and a licensed clinical social worker to lay the foundation for quality ongoing care. This care assists an older individual with the ability to enjoy good health, improve overall quality of life, reduce the need for hospitalization, plan for long-term care, as well as live independently for as long as possible.

Staff members are trained in geriatric care and patients are given complete cognitive and physical assessments in addition to the specific medical treatment called for upon hospital admission.
