Offers a variety of services designed to improve or support the consumer's adjustment and integration within the family and community through rehabilitative activities, social skills development, recreational or educational programs, activities of daily living, skills development, and other supportive services. Specific programs include: adult developmental training, family support services (FSS), clubhouses, and drop-in centers.

Provides care and companionship for seniors 60 years of age and older, who need assistance or supervision during the day.

Comprehensive, short-term outpatient program for adults with emotional difficulties with or without substance abuse. provides intensive treatment in a structured environment for up to 6 hrs per day.

Transitional Outpatient Services offers a multi-modal, multi-disciplinary structure for a variety of programs. The program has a variable length of treatment and offers flexibility in scheduling as symptoms are alleviated and the individual is able to resume their typical activities. Transitional Outpatient Services is more intensive than traditional outpatient and is designed to achieve stabilization and resolution of psychiatric and chemical dependency conditions. This level of care is specifically available for those adult clients who do not require the intense level of psychiatric care provided by inpatient or partial hospitalization programs.

Offers a full range of home care services administered by our skilled professional nurses and therapists in conjunction with a physician's plan of treatment. In the comfort of home, patients can receive personalized care designed to improve their quality of life, restore health, promote independence and reduce hospital readmissions.

Provides specialized Alzheimer's Disease and Related Dementias (ADRD) care and supervision in a protective secure setting. Services include medication assistance, individualized care plan, health monitoring, nutritious meals and snacks, mind stimulating activities, exercise programs, community outings and projects, pet and music therapy.
Also offers both drop-in and respite care when slots are available.

The Adult Autism Waiver (AAW) is a 1915(c) Home and Community-Based Services (HCBS) Medicaid waiver designed to provide long-term services and supports for community living, tailored to the specific needs of adults aged 21 or older with ASD. The program is designed to help adults with autism spectrum disorder participate in their communities in the way they want to, based on their identified needs.

The Caregiver Support Program (CSP) provides resources and assistance to caregivers who are either:
- 55+ and caring for a related child or a related disabled adult
- Any age and caring for a functionally dependent individual of any age with Alzheimer's Disease or a related disorder

The CSP provides eligible primary caregivers of functionally dependent individuals aged 60+ with reimbursement for out-of-pocket costs associated with caregiving, such as respite care, incontinence supplies, and home modifications.

Grandparents and other older relatives aged 55+ who are the primary caregivers for related children may be eligible to be reimbursed for day care, camps, seasonal clothing, school supplies, tutoring, and registration fees for school activities or sports.

Protects older adults who are at risk of harm because of any type of abuse by insure that the basic health and safety of all older adults are met in the least restrictive setting possible, and that no older adult should remain at imminent risk to person or property. Responsible for investigating any referral regarding safety concerns for an older person within 72 hours.
If the older adult is being abused, the agency offers services to stop the abuse and prevent it from occurring again.

Provides structured activities and health related services. Provides social and mental stimulation, to care for their physical needs. Give their caregiver, usually a family member, a much-needed break. Services will be provided in the Hawley Senior Center.

Services include assistance with daily activities that help maintain older persons in their own homes by arranging for an agency that provides aides to assist with bathing, personal care, dressing, grooming, and respite care.
Services also include adult day care consisting of a supervised day program which includes activities, socialization and nutrition services provided by a state licensed, non-residential facility for older adults.

Helps county seniors to access the supports they need.

The Meadows at Melmark is a work-centered program for adults with mild-to-moderate disabilities. Participants live with friends in staff-supervised, single-family homes, or in the community with their families.

Adult Day Workshops provide services for adults with moderate-to-profound developmental and physical disabilities, based on individual needs and abilities. For adults with lower cognitive levels or those who are non-ambulatory, activities include multi-sensory stimulation, massage therapy, hand-over-hand training, and personal assistance.

Provides a daycare center for older adults who are not capable of full-time independent living. Services may include basic medical services, nursing services, nutrition, occupational therapy, physical therapy, recreation, transportation and more intensive personal care but vary by location.

The Ombudsman program is designed to support and empower people by resolving individual complaints involving long-term care service, while working to improve and enhance the long-term living system for the residents and their families.
Ombudsman advocate for and protect the rights of older adults receiving or denied long-term care services in nursing homes, assisted living, adult day centers, domiciliary care homes, personal care boarding homes, or their own homes.

Offers adult Daycare. Assists with services provided daily that include: Nursing Supervision, Health monitoring; Therapeutic activities, Individualized Care Plans, Physical exercise, Games, arts and crafts, Entertainment, Nutritious hot meals, Personal Care, and Snacks.

Advocates for the rights of individuals. Investigates and resolves complaints from seniors living in long-term care facilities, including nursing homes, domiciliary homes, assisted living, personal care homes, adult day care, and in-home long-term care services. Offers representatives who visit the residents in their homes and answer questions about billing, concerns about a long-term care service provider, and appeals regarding transfers, discharges, discontinuance, or changes in services.

-Nursing Home Assessment: Any person age 18 and older who desires placement in a nursing facility, personal care home, or domiciliary care home can be assessed by the Care Management staff of the Adams County Office for Aging, Inc.
-Domiciliary Care (Dom Care): Provides adults 18 years or older, who cannot live independently, with sheltered living in private homes with a family-like atmosphere.
-Personal Care Homes: Sometimes called assisted living facilities, offer room and board and assistance with daily living activities.
-Adult Day Services: Provides functionally disabled older persons with an array of activities in a protected environment which will enable them to maintain independence within the community.

Services include a daily congregate meal program, health screenings, recreational and socialization programs, educational seminars, wellness programs, driver's education courses, food services, day trips, and more. Provides information and referral to connect individuals to additional services as needed. Operates as a cooling station for those who cannot adequately cool their homes or do not have a place to cool down.

Services may include social and recreational activities, training that is essential for sustaining activities of daily living, hot meals, as appropriate and, if an adult day health care center, health and related services.

Provides short-term intensive treatment those who experience a mental health crisis but do not require inpatient hospitalization. Provides individual and group treatment for up to 6 hours per day, five days per week.

Acute psychiatric for children & adolescents; psychiatric residential services for children & residential, intensive and community-based treatment, special education day school programs, psychiatric services, and school-based services.

Champions a life of inclusion, opportunity, and equity for people with intellectual and developmental disabilities through advocacy, education, and support.

Offers adult substance abuse counseling on an outpatient and intensive outpatient basis. Provide a comprehensive assessment, referral,drug and alcohol education, and individual and/or group counseling.

Also provide aids to prevent addiction in the first place.

Offer Medication Assisted Therapy (MAT) for Opioid Addiction. Pysicians use Suboxone and Vivitol at treatment modalities.

Day Hospital/Intensive Outpatient mental health treatment program for those who have a diagnosis of mental health illness and who are experiencing serious to severe levels of emotional distress. Program is designed to allow individuals to remain with their families and community. Program varies from 3-6 hours and includes the following services:
* psychiatric evaluation
* individual therapy
* group therapy
* psych-education regarding symptom management, coping skills development and problem solving
* Psychopharmacological consultation
* Treatment includes the participation of family members and other significant persons
* Individual meetings to review treatment
* Discharge planning, including aftercare programs.
