Transitional Outpatient Services is more intensive than traditional outpatient and is designed to achieve stabilization and resolution of psychiatric and chemical dependency conditions. This level of care is specifically available for those adult clients who do not require the intense level of psychiatric care provided by inpatient or partial hospitalization programs.

The clinician will help you identify areas in your life that you would like to change and help you develop a plan to make these changes.

The clinician will arrange for you to see a PATH psychiatrist, if indicated.

The CARE program is a school-based, partial hospitalization program that provides a clinical and alternative education environment for children, ages six through ten years old, who have been diagnosed with a significant mental health disorders. Licensed by the Department of Public Welfare, program provides an integrated therapeutic and educational program for children and adolescents in grades 6-12.

Offers a Medication-Assisted Treatment (MAT) program for those suffering from substance use disorders. The program is a combination of both FDA-approved medications and counseling. Three types of MAT are offered:
Vivitrol: a monthly opioid blocker for patients free of opioids for at least five days. We can provide an outpatient detox protocol if needed.
Sublocade: Monthly Buprenorphine injection. Patients need at least seven days on oral Buprenorphine which is provided.
Probuphine: Six-month Buprenorphine implant for patients stabilized on 8mg or less of oral Buprenorphine.
Please note, our practice does not prescribe maintenance oral Buprenorphine.
