Hospice Care for patients who have life-limiting illness or health conditions. Hospice care is available in the patient's own home or in the facility where the patient resides.

Hospice manages symptoms to maintain or enhance quality of life. Hospice services are available for anyone with a life-limiting illness who wants comfort care rather than curative procedures. Hospice care may span months or more and can be at home or in a nursing home. Hospice also pays for additional items that are related to the patient's terminal illness such as: medications, equipment and other personal comfort items.

The Gatehouse is a seven-bed short-term inpatient Hospice unit for the management of pain and other symptoms relating to a life threatening illness. In addition, patients who are struggling with discomfort may come to the unit for treatment, then return home or other living arrangements. Families can spend quality time with patients in private bedroom suites, a comfortable family room, a kitchen and a meditation room - all while patients receive around-the-clock, skilled nursing care to manage their difficult symptoms.

Provides a unique tele-health monitoring service to patients in their homes. This daily home monitoring equipment is a small machine placed in a patient's home that collects an individual's vital signs such as weight, blood pressure, oxygen saturation, heart rate and temperature. Each day a calming voice guides the patient through a simple procedure to collect this data and then sends this information to nurses at Home Care & Hospice for evaluation. Nurses notify the patient's physician if there are any changes in condition so problems may be addressed early - preventing an emergency room visit or hospital admission.

This program is for patients with a life limiting illness or terminal disease. Nurses work with patients who are still receiving aggressive disease treatment and are not yet ready to embrace the Hospice philosophy.

Abington Jefferson Health's Kind Pediatric Hospice program is committed to providing compassionate and comprehensive care to children and their families for serious illness through the end of life.

Our inpatient unit, which offers specially designed pediatric rooms and a dedicated family lounge, is devoted to children from newborns to young adults. The staff provides care for children in the advanced stages of illness, including cancer, congenital conditions, neurological diseases, genetic conditions, HIV/AIDS and traumatic injuries. Abington Health also offers in-home pediatric hospice services.

Provides in home care plans that may include personal care and companionship-homemaker services, people with terminal illness, and caregivers needing planned respite hours. Help is available 24 hours a day, 7 days a week.
