Case management help older adults to assess their abilities and needs, explore options, develop plans to best utilize their abilities, and arrange for informal and formal supports (in-home services) to meet their needs. With an older adult looking for placement, they can assist older adults who need to enter a nursing home or personal care facility with determining the type of residential placement that will best meet their needs, and with securing such placement.
