Provides supported living services to individuals with mental health needs through medication monitoring and in-home assistance. In-home assistance includes budget and shopping education, nutritional education, household maintenance support and life skills

. Supported Living Services include, but are not limited to budgeting and shopping assistance, nutrition education, household maintenance support, and life skills assistance.
Eligibility: Individuals with serious mental health and/or intellectual disabilities.
