Provides free primary medical care by appointment for uninsured or under-insured adults.
Volunteer opportunies available: Physicians, CRNP, LPN, RN's and clerical volunteers
Eligibility: Lebanon county residents who are uninsured and under-insured adults (18 years old and older (who are 200% below the federal poverty guidelines.
Hours: By appointment Monday, 9:00am to 3:30pm
Tuesday, 9:00am to 3:30pm
Wednesday, 9:00am to 3:30pm

Intake Procedure: Call for appointment; Walk-in patients will be required to sign an income self-certification that their income is at or below 200% of the federal poverty guidelines.
Intake Requirements: Picture ID, proof of residency, and income verification from each household member earning wages.
