Provides funding and administrative oversight for services in our community that support people and their families living with intellectual disabilities.
Eligibility: Lebanon county resident with diagnosis of intellectual disability based on results of objective standardized testing. Diagnosis of intellectual disability requires that both a person's general intelligence and ability to function in daily life are significantly below average. These two conditions must be present at birth or occur prior to person's 22nd birthday.
Hours: Monday through Friday, 8:00am to 4:30pm

Intake Procedure: Contact office to schedule intake appointment
Intake Requirements: Psychological evaluation, assessment of adaptive functioning, birth certificate, social security card, insurance card. Intake worker can help family obtain documents if not available
