UPMC - Home Care and Hospice - Home Monitoring Equipment
Provides a unique tele-health monitoring service to patients in their homes. This daily home monitoring equipment is a small machine placed in a patient's home that collects an individual's vital signs such as weight, blood pressure, oxygen saturation, heart rate and temperature. Each day a calming voice guides the patient through a simple procedure to collect this data and then sends this information to nurses at Home Care & Hospice for evaluation. Nurses notify the patient's physician if there are any changes in condition so problems may be addressed early - preventing an emergency room visit or hospital admission.
Hours: Weekdays, 8:00am - 5:00pm; Saturdays, 8:00am - noon
