Services include medical or psychological evaluations, vocational testing and counseling, payment for medical items and procedures such as wheelchairs, prosthetic, orthotics, hearing aids, surgical procedures and physical, occupational and speech therapy.
Eligibility: Individuals 14 years old or older who have a medically documented cognitive, mental, or visual disability.
Hours: Monday through Friday, 8:00am to 4:30pm

Intake Requirements: If available, medical documentation from board certified physician or other medical specialist verifying proof of cognitive, mental, or visual disability; otherwise documents can be requested by signing medical release form upon intake process for program determination
