Service assists residents to connect with necessary resources that will enable them to achieve and maintain self sufficiency.
Eligibility:
B) Individuals must be residents of Luzerne or Wyoming County.
C) Individuals must be referred by Luzerne/Wyoming County MH/DS. If a potential individual is not specifically active with Luzerne/Wyoming County MH/DS, then the referral will need to be approved by Luzerne/Wyoming County MH/DS.
