SAM - Schuylkill - Transition Age Youth Housing Project
Assists individuals in securing financial resources, applying for pertinent documents, searching for housing and applying for housing. In some cases, multiple months of rent may be paid along with a security deposit. Other supports, include furniture, household supplies, payments for accessing documents and other unique needs.
Eligibility: Persons age 18 through 25 years of age who are homeless, at-risk of homelessness or living in substandard housing conditions. Applicant must be enrolled in Service Access and Management, Inc. or enrolled in the PATH Program and must have a diagnosed mental health illness and be enrolled in Medical Assistance.
Hours: Monday through Friday, 8:30am to 4:30pm
