Eligibility:
Must be at least 18 years old, must be able to be named on a lease
as a responsible party, must have CCBH (Medical Assistance)
Insurance, must have a diagnosis of a serious, persistent mental
illness and/or substance abuse, must have some source of verifiable
income within the household.

Intake Procedure:
Applications for assistance are accepted during open enrollment
periods, approximately 4 times per year. Emergency applications are
accepted at any time.
Intake Requirements:
Applications must be completed and submitted with one of the
following: a Blended Mental Health (or) Drug and Alcohol Case
Manager, a Mental Health Treatment provider, or a Substance Abuse
Treatment provider.

Helpful Tips:
During open enrollment periods, applications are reviewed and
approved according to specific Priority Categories of Need. These
are:
*homeless- street or place not meant for human habitation
*shelter
*state hospital discharge
*Reading Hospital or Berks Counseling Center ACT Team participant
*transitional housing/group home discharge
*either diversion form or release from Prison System, within past 60 days
*transitional aged youth (18-21), transitioning from CYS or JPO Systems including residential or foster care placement
*hardship- paying more than 50% of gross monthly income towards rent and basic utilities and under a current, official eviction
*rooming house resident
*lottery- paying more than 50% of gross monthly income toward rent and basic utilities (approved lottery applications are included into a lottery "draw" for random selection at the end of each open enrollment period
