Intake Procedure:
Dial 2-1-1: Individuals/families must have a referral sent by a
housing specialist to receive these services. Once a referral is
received, a call, within (3) business working days, will be placed
directly to the individual to follow up on the current information
and determine if the individual meets the eligibility for services.
At that point, an intake appointment is scheduled with the
individual. Individuals will need proof of income for the
household, documentation of serious mental illness, amounts of
monthly bills, and eviction notice or lease if seeking security
deposit assistance.

Helpful Tips:
Assistance is available until funds are depleted. Emergency
supplies for individuals experiencing homelessness with no
identified shelter include: tents, sleeping bags, flashlights,
first aid supplies, camping utensils, hygiene items, backpacks,
non-perishable food items, etc.
