Eligibility:
Children age birth to three (3) are eligible to receive an
evaluation if their parent(s)/guardian(s) have concerns regarding
their development. Infants/toddlers are eligible to receive early
intervention services when they are determined, through evaluation,
to have a 25% or greater delay in one or more of five developmental
areas (i.e.: Cognitive development, language/speech development,
social/emotional development, Physical development and/or self-help
development).
A child is eligible for at-risk tracking services if they meet one (1) of the following criteria:
1. Birth weight under 3.5 pounds
2. Cared for in a neonatal intensive care unit (NICU)
3. Born to chemically dependent mother and referred by a physician, health care provider or parent
4. Abused or neglected, as substantiated and referred by the county children and youth agency under Child Protective Services Law
5. Confirmed dangerous levels of lead poisoning as set by the Department of Health
6. Experiencing homelessness

Intervention Service Coordination in Franklin/Fulton provided by
