Provides medical care and pharmaceutical services. Offers complete health assessments; screenings and diagnostic testing; diagnosis and management of mild to moderate chronic diseases including: hypertension, diabetes, elevated cholesterol, CAD, COPD, and arthritis; minor trauma care; free medication (except controlled medication); referrals to specialists within the Penn Highlands network; and more.
Eligibility: Adults age 18 and older. Residents of Clearfield, Elk, and Jefferson counties. Currently Employed. Medically uninsured. Household income at or below 300% of federal poverty level.
Hours: Monday through Wednesday, 9:00am to 5:00pm; Thursday and Friday, 9:00am to 12:00pm
