Ridgway Facility - Blended Case Management
Offers behavioral health consumers that are active in their treatments to utilize a case manager to identify and link to available community resources, coordinate various services they may have and to monitor their overall achievement on their recovery goals.
Eligibility: Adults and children with a primary diagnosis of mental illness.
