Eligibility:
Individuals who are between 14-18 years of age, residents of
Crawford, Venango, or Mercer County, have the presence of, or
history of, mental health concerns, and choose to receive services.
Hours:
Services will be delivered outside of school hours and will vary
with days and times to meet the needs of the individual.

Intake Procedure:
Anyone can make a referral (including self-referrals).
Intake Requirements:
Enrollment requires a recommendation from a Licensed Practitioner
of the Healing Arts. A LPHA is defined as: Psychiatrist, PCP,
Physician Assistant, Certified Nurse Practitioner, or Licensed
Psychologist.
