Meadville Offices for Crawford County Human Services - Intellectual Disabilities Supports Coordination
Locates, coordinates and monitors services for qualified individuals with an intellectual disability and / or an autism spectrum disorder. Acts as an advocate for individual and family rights and freedom of choice. Ensures quality programming and the development of an individualized plan and support network. This includes, but is not limited to, family support, daily living activities, recreational activities, supported employment, residential placement and referrals to a variety of community resources.
Eligibility: Individuals with an intellectual disability (full scale IQ score of 70 or below) diagnosed before age 22. Individuals with an autism spectrum diagnosis who have significant deficits in day-to-day functioning.
Hours: Monday through Friday, 8:30am to 4:30pm
