Intake Procedure:
Contact Assistive Technology Resource Coordinator to complete an
"Information & Assistance" intake
Intake Requirements:
Name, address, phone number, date of birth, social security number,
emergency contact, and technology requested.
Geographical Area:
Clarion, Crawford, Erie, Forest, Mercer, Venango, and Warren
counties
Helpful Tips:
Contact CRI Center for Independent Living for assistance advocating
for services.
Facility/ADA Access:
CRI Erie Office is completely accessible to people with
disabilities.
