Residential Services - Long Term Structured Residence
Offers a recovery-focused, mental health treatment program for people transitioning from an inpatient mental health stay back into the community. Includes these programs Blended Case Management, Psychiatric Rehabilitation, Mobile Medication Management, Certified Peer Support, Assertive Community Treatment, and other resources as needed.
Eligibility: Individuals who are 18 years and older who have been diagnosed with a serious mental illness (e.g. bipolar disorder, schizophrenia). Additional criteria, call for more information.
