Eligibility:
Erie County resident 18 years and older; diagnosed with a serious
mental illness; history of multiple psychiatric hospitalizations or
a recent inpatient placement with an unsatisfactory outcome; can be
safely diverted to a less restrictive level of care than a hospital
setting but who requires a 24-hour, supervised setting; lacking
community and primary supports. Other requirements may be needed,
call for full details.
