Main Administrative Office - Blended Case Management Youth
Provides children, transitional youth, and parents a child-centered service designed for children and adolescents who have, or are at risk, for a serious mental illness or emotional disturbance, get the treatment and support necessary to help them succeed in the home, at school, and in the community.
Eligibility: Serves Erie County residents
Child service requirements: up to age 18 or up to 21 if the child is still in school and has an individualized education plan (IEP) and enrolled in school. Must have a mental health diagnosis. Visit the Eligibility Website to download the brochure for additional eligibility information.

Transitional Youth requirements: age 18-24 who meets criteria for either child or adult eligibility based on age and school status. Visit the Eligibility Website to download the brochure for additional eligibility information.

Parents requirements: adult over 18, has a diagnosis of a serious mental illness (e.g. schizophrenia, bipolar disorder); recent psychiatric hospitalization or frequent contacts with Crisis Services; and significant difficulty independently management responsibilities at home, work, in the community.
