Erie - Blended Case Management
Helps people with special mental health needs coordinate community-based psychiatric, medical, social, and educational services.
Blended Case Managers help those who have or are at risk for developing a serious mental illness or emotional disorder get the best treatment and support available in our area.
Eligibility: Any child, adolescent, or adult resident of Erie County that is in need of a high level of mental health service. Examples include but are not limited to: recent mental health hospitalization, involvement with child welfare services, placement in a residential treatment facility or significant difficulty managing life/social stressors.
Hours: Monday, Tuesday, Wednesday, Thursday, and Friday, 8:00am to 5:00pm; Crisis Services operates 24-hours/day, 7 days/week
