Friendship Circle Senior Center - Care Management
Assesses client's needs and helps client to access community-based services and resources. Arranges for in-home support, such as meal preparation, assistance with personal care, skilled counseling, house and yard work, transportation to medical appointments, and the reassurance of a personal emergency response system.
Eligibility: Residents of Delaware County age 60 and older; eligibility will be determined during home visit.
Hours: Monday through Friday, 9:00am to 4:00pm
