Intake Procedure: Sign up during available hours. Walk-in only, no appointment necessary.

Intake Requirements: Applicants must have all necessary documentation with them at the time of sign-up in order to complete an application.
A) Identification is needed for each individual in the household: Adults (ages 18 & older) – Photo ID OR birth certificate; Children (ages 0 – 17) – Birth certificate, medical card, OR other document verifying name and date of birth.
B) Official proof of address: A current utility bill, a piece of official mail (no older than 60 days), or a copy of your lease. We will not accept IDs as proof of address.
C) Gift information for each child between the ages of 0 and 14: Two gift ideas each up to $25 in value; Correct clothing sizes for each child – shoes, shirts, dresses, and pants.
D) Proof of income for everyone living in the home
