Northeast Counseling Nanticoke - Joint Planning Team
Uses High Fidelity wraparound, a process developed as a way of multiple systems coming together with a child and family and creating a highly individualized plan to address complex emotional issues. The key characteristics of the process are that the plan is developed by a family centered team, is individualized based on the strengths and culture of the child and their family and is needs rather than services driven. It allows the youth and family to identify their unique needs and develop an individualized plan guiding them to recovery.
Eligibility: Residents of Luzerne and Wyoming Counties age 5 to 21 with complex emotional and behavioral health issues, with multi-system involvement, and who are at risk for out of home placement.
Hours: Monday through Friday, 8:30 am to 4:30 pm
