- interpret clinical findings and act as liaison between health care professionals
Eligibility:
Any individual of any age who has a disability may request
services. The disability had to have manifested before a person is
22 years of age. It may include a physical or mental delay or a
combination of both.

Monday through Friday, 8:00am to 4:00pm

Intake Procedure:
Referrals can be made by family member, health care professional,
community agency, or friends. After referral, nurse will obtain
permission, visit home for intake and initial assessment. Nurse
will work with individual and family to create a plan which may
include referrals, follow-up visits, and home visits as needed.
