Provides community-based prevention services with the goal of diverting families from entering into formal child welfare services. Services include strength and needs assessment; identification and implemention of problem-solving interventions; and when appropriate, advocacy, arrangement, coordination, monitoring, and evaluation of support services to meet client families' needs.
Eligibility: Children under the age of 18 living in the home

No active Department of Human Services cases
Families should not be presently receiving other case management services in the home.
Hours: Monday through Friday, 8:30am to 5:00pm
