Eligibility: A) Adults 18 years and older.
B) Residents of Carbon, Lehigh, Monroe, Northampton and Pike Counties.
Hours: Monday through Friday, 8:30am to 4:30pm; on-call support 24/7.

Fee Amounts: Provider of the Community Health Choices (formerly Medicaid Waiver program) as well as a provider for the Veterans Administration. Assistance at no cost with providing the resources on how to determine eligibility for said programs.
Intake Procedure: Call or visit website to apply. An in-home assessment will be completed prior to start of care.
