Provide assistance in finding housing, food, transportation, and other community resources. Other services include helping make a connection for utility assistance, applying for health insurance, patient advocacy, managing nursing care, and the coordination of patient care.
Eligibility: Serves individuals or families wiho are uninsured or have commercial insurance, Medicaid, or Medicare.
Hours: Monday through Friday, 7:30am to 5:00pm
