Scranton School-Based Practice - Health Assessments
Primary care team offer health assessments for the whole family. During an annual visit, team will perform a routine physical examination, discuss any medications or test results and share additional resources (i.e. nutrition counseling, smoking cessation) that can supplement your care.
Eligibility: Open to all ages.
Hours: Monday through Friday, 8:30am to 5:00pm

Fee Amounts: Accepts most insurance plans, including Medical Assistance (Medicaid), Medicare, Highmark Blue Cross Blue Shield, CHIP, and more. We operate as a safety-net provider of care, focused on delivering right-venue, high-quality, affordable health services to all regardless of insurance status. If a patient does not have health insurance, our team is ready to connect them to health benefits coverage options, including our sliding-fee scale. Eligibility for our sliding-fee scale is based on family (or household) income and size and provides discount levels for qualifying patients.
