Administrative Headquarters - Nurse-Family Partnership
Provides an evidence-based program to support pregnant people in which registered nurses conduct home visits throughout the pregnancy and continue until the child turns two years old.
Eligibility: A) Women must be income-qualified (low-income)
B) Must enroll in the Nurse-Family Partnership program by the 28th week of pregnancy.

The program prioritizes first time parents.

Intake Procedure: Referral may be made by another agency, provider, or client may make self-referral. Intakes are scheduled upon receipt and may be done by phone or in person, on site or in the home.
