Upper Darby - Transition Age Youth (TAY) Division
Assures appropriate services are available for transition-age youth who are experiencing mental health challenges. Contracts with agencies who provide an array of services and resources which are focused around trauma informed principles, recovery, resilience, and self-determination. Services include high fidelity wraparound, outpatient treatment, blended case management, transition age housing, and program of assertive community treatment.
Eligibility: Delaware County Residents age 18-26
Hours: Monday through Friday, 8:30am to 4:30pm
