Consortium - Children's Blended Case Management
Provides individualized in-community support for children (up to 17 years of age) with serious and persistent mental illness. Services include a 24 hour-a-day, 7 day-a-week crisis response. The program uses a community support model and for consumer care and assists with coordination of community mental health needs.
Eligibility: Children up to 17 years of age with mental illness currently placed in residential facilities, and those in the community with moderate needs.
