Provides primary care through physical exams, responding to acute illnesses such as the flu or sinus infections, and helps patients manage chronic diseases such as diabetes and high blood pressure.
Eligibility:
  • Household income at or below 250% of poverty level
  • Must reside or work in Centre County
  • Must not have insurance on services in which you are applying
Hours: Monday through Thursday, 8:00am to 4:30pm
Fridays, 8:00am to 4:00pm

Helpful Tips: If a CVIM patient needs an out-patient test (x-ray, ultrasound, MRI) or an out-patient referral, the nurse will schedule the procedure at either the Mount Nittany Medical Center or the Geisinger clinic in Gray’s Woods.
