Permenant Supportive Housing
Intervention that combines affordable housing assistance with voluntary support services to address the needs of chronically homeless people. Referrals for services are made through Coordinated Entry System.
Eligibility: Individuals must have a Mental health diagnosis, be homeless and willing to live with others. Documentation is a must.

For Families, one member of the household must be disabled and be able to provide documentation, as well as be homeless.
Hours: Monday through Friday, 9:00am to 4:00pm
