Visit in person for assessment of eligibility and to apply.
Intake Requirements:
A) PA State ID for all household members age 18+
B) Social Security Cards for all household members
C) Proof of all household income
D) Documentation of past due rent from landlord
E) Proof of eviction, if applicable
F) Copy of lease
