Broomall Office - Transition to Independence Program
Provides support to young people experiencing emotional and or behavioral struggles as they plan their own future across five transition domains: Education Opportunities, Living Situation, Employment and Career, Community Life Functioning, and Personal Effectiveness and Wellbeing. The program uses an evidence-supported model developed to engage and support young people through relationship development, person-centered planning, and a focus on the young personâs future.
Eligibility: Residents of Delaware County ages of 16 to 26 with emotional and behavioral difficulties who have a primary psychiatric diagnosis and are covered by Medicaid (Medical Assistance)
