Judith Creed Horizons for Achieving Independence
Transitions Program
Provides activities and training after school and on weekends for young adults to help them be prepared to live and work a supported, independent life. Activities include social activities, game nights, resume drafting, film club, group counseling, and more.

Also supports and educates the member's family to anticipate the "letting go" process.
Eligibility: Serves residents in Philadelphia with disabilities age 18 to 30.
Hours: Office Hours: Monday through Friday, 9:00am to 5:00pm
