Low-income family; expectant mother or parenting a child birth
through five years of age

Monday through Friday, 8:00am to 5:00pm

Intake Procedure:
Initial referral completed via phone call or one page referral;
faxed to office. In person application with documentation of
household size and income to meet income eligibility requirements.
