Designed to promote a healthy lifestyle through nutrition to help manage heart disease and diabetes safely and effectively. Meals will be delivered to all participants.
Eligibility: Individuals with a primary contributing diagnosis related to cardiac and/or diabetic issues.

Fee Amounts: To be negotiated with the primary insurance carrier; After the initial 90 days, participants who elect to continue receiving the beNourished meals will pay privately for the program.
Intake Procedure: Must be referred by: doctor and/or health care provider.
Intake Requirements: Participants are required to sign an agreement that they will eat the meals and follow the nutrition recommendations made by the doctor, dietitian, and community health educator. Participants will be monitored for health benefits through phone calls and in person visits.
