Medical Case Management
Offers support to promote family stability in coping with the unique challenges of living with HIV/AIDS. Educates the community to prevent the transmission of HIV and to reduce the stigma associated with HIV and AIDS. Provides the following services:

  • Infectious disease clinical practice staffed by a physician board certified in infectious disease.

  • HIV Education/Prevention and Testing Services

  • Pre-exposure prophylaxis (PrEP). Medication prescribed to high-risk, qualifying individuals to reduce the chance of HIV transmission

  • Confidential, client-centered case management

  • Medication assistance/health insurance support

  • Transportation/financial assistance for: housing, doctor visits, utilities, dental care, and etc.

Helpful Tips: Rapid HIV results are provided in as little as 15-minutes with Chemobio HIV 1/2 STAT-PAK, as well as chlamydia and gonorrhea testing.
