Clinic provides Primary Health Care, Family Practitioners, Care Management, an initial behavioral health assessment, brief counseling services and referrals to outpatient providers when needed.
Eligibility: Care for patients during every stage of life, from infancy through adulthood and into older age.
Hours: Monday, Friday, 7:00am to 5:00pm; Tuesday, Thursday , 8:00am to 6:00pm

Fee Amounts: Sliding Fee Discount Scale program is designed to assist individuals and families struggling to pay for health services. No patient is refused because of a lack of financial means. Regardless of your plan, level of coverage or ability to pay, we will provide care to all those who seek it.
