Community Clinic
Assists the uninsured with access to health care through a network of volunteer physicians and services provided by four area hospitals. Patients are treated at the clinic and referred to the area hospitals if further testing is needed. Provides primary care, chronic care, physicals for jobs/school and referrals to specialty physicians.
Eligibility: Residents of Montour, Northumberland, Snyder or Union County.
Must be a US citizen, between the ages of 19-64.
Have no form of health insurance and meet income eligibility.
Hours: Monday through Friday, 8:30am to 4:00pm

Intake Requirements: Must bring photo ID and proof of income. Financial eligibility is determined by a percentage of the Federal Poverty Level.
