Offers medical care collaboration with local institutions, universities and volunteer providers. Our interdisciplinary model is based on collaborations between our community nurse liaison and two medical co-directors, a physician and a nurse practitioner.
Eligibility: No limitations or restrictions
Hours: Mondays 12:30 p.m. to 2:30 p.m. and 6:00 p.m. to 8:00 p.m.
Wednesdays 6:00 pm to 8:00 pm
Thursdays 10:00 a.m. to 12:30 p.m.
