PATH
Philadelphia - Continuity of Care Program
You are served if you are a child, adolescent or adult experiencing mental health and/or substance use problems. When you first contact PATH, you will speak with a clinician who will listen to your concerns and schedule an initial intake assessment with you. At the intake appointment, you will meet privately with a professionally credentialed clinician to complete a comprehensive assessment which usually takes about 2 hours. The clinician will ask you questions about your life and help you identify those areas you would like to change. You and the clinician will work together to identify your strengths and challenges and define the services at PATH that will best meet your needs. The clinician will arrange for you to receive those services and see a PATH psychiatrist, if indicated.
