VNA Personal Navigator Program
Provides public benefit navigation and application assistance for enrollment in the following public benefits: Medical Assistance, PENNIE, Medicare related supports, prescripton plans, supplements and subsidies, PACE/NET, One Sight, SNAP, Free/Reduced Cost School Lunches, CCIS, Senior Farmer's Market Vouchers, TANF, PECO subsidies, LIHEAP, Free cell phone program, the PA Rent and Property Tax Rebates and other federal, state or local public benefit programs.
Eligibility: Residents of Montgomery County or the Greater Phoenixville Area
Hours: Monday through Friday, 8:30am to 5:00pm
