A) Physical disability and/or experience working with persons who have physical disabilities
B) lived experience managing behavioral health challenges.
Services may include building a network of natural supports; developing a Wellness Recovery Action Plan; facilitating collaboration with service providers, healthcare providers, transportation, and other resources; and assistance with nursing home transition planning.
Eligibility:
Residents of Philadelphia age 21+ who have a physical disability
and a mental health diagnosis, and who are capable of meeting with
a Journeys Peer (face-to-face and over the phone)
Hours:
Office Hours:Monday through Friday, 9:00am to 4:00pm
